I practice as a "fee for service" physician or also known as "out of network provider" so that I can offer my personalized, comprehensive and preventative care to you. Fees for the office visit are requested at the time of service and we submit a claim to your health plan for reimbursement on your "out of network" benefits. Your expense for the office visit fee can also be applied toward FLEX health care accounts with your employer.
